Name Changes

Required Documents indicating your new name:

  1. Social Security Card (not the application for a card)
  2. Driver’s License
  3. OR a U.S. Passport
  4. Original

Bring the required documents to the Human Resource Development Office to complete the following:

  1. NM Educational Retirement Form 42 (needs to be notarized)
  2. US Department of Homeland Security Form I-9
  3. LCPS Personnel Directory Data Form
  4. W-4 Employee’s withholding
